Your membership is good for one year from the start date. The start date of your plan is your enrollment date. We will send you a reminder one month prior to your membership expiration date.

Basic benefits include diagnostic & preventative services at no charge. 

• Two routine cleanings or perio maintenance per year
• Check-up exams
• X-rays, including one set of bitewings, and any other x-rays needed
• Flouride

You will also receive 20% off all other dental services, including:

• Periodontal services
• Fillings
• Crowns
• Bridges
• Complete and partial dentures
• Veneers
• Whitening

Each adult is $350. Each child 12 and under is $250.   Payment must be made in full prior to receiving any treatment.
